Young Adult's Camp Registration Form

Join us for this years young adults camp for a weekend of good fellowship, good food and good teaching. We'll be hearing from Heather Packett, Teaching Pastor at Crossway Baptist Church, as she explores the life of Moses and what it means for us to be 'Faithful' followers of Jesus today.

 

We have once again extended the invitation to other young adult communities in our area, and this year, we're excited that Crossway Booroondara have accepted the invitation and have decided to join us for the weekend.

 

We don't want cost to be a barrier for anyone! End every effort has been made to keep costs per person as low as possible, if you require assistance or would like to set up a payment plan please get in touch with Kane by email (kanel@sthils.com) as soon as possible!

Camper Details

We are going to have pizza together 6pm Friday night for those that can make it down in time for dinner. Please indicate below if you’ll be present for dinner so we know how many pizzas to order! There will be a $5 contribution if you are able.

MEDICAL CONSENT

I authorise the Leader in charge of any activity during the Young Adults Camp to consent on my behalf, where it is impractical to communicate with an emergency contact person, for me to receive any medical, surgical or dental treatment as may be deemed necessary. I also agree to indemnify St Hilary’s for the cost of any medical treatment deemed necessary.  

CONSENT TO TAKE AND USE PHOTOGRAPHS

I understand that there may be photographs and/or video footage taken during these activities and am willing to be so filmed in appropriate settings. I am also willing for these photos or footage to be used to promote the ministry in a way that does not identify my name and details and are not published on a website or distributed in an electronic format without my consent.

Consents
