Revive Winter Camp Registration Form

Join us for this year’s REVIVE winter camp as we explore the theme of BATTLEFIELD which will take us through Paul's letter to the Church at Ephesus..

Youth can look forward to fun games and outdoor activities, vibrant worship music, inspiring talks, and faith-filled conversations!

 

Year 6-12 are at camp for 4 full days and this year transportation from St Hilary's Kew Campus will be provided.

Drop Off: 8:00 am Monday

Pick Up: 4:00 pm Thursday

MEDICAL CONSENT

I authorise the Leader in charge of any activity during REVIVE Winter Camp to consent on my behalf, where it is impractical to communicate with me, for me to receive any medical, surgical or dental treatment as may be deemed necessary. I also agree to indemnify St Hilary’s for the cost of any medical treatment deemed necessary.  

CONSENT TO TAKE AND USE PHOTOGRAPHS

I understand that there may be photographs and/or video footage taken during these activities and am willing to be so filmed in appropriate settings. I am also willing for these photos or footage to be used to promote the ministry in a way that does not identify my name and details and are not published on a website or distributed in an electronic format without my consent.

After 12 June, cost is $370 for the first child in a family and $330 for subsequent children.

Cost per team member is $250 unless an alternative arrangement has been discussed with a member of the ministry or finance team

Consents

  • I consent to my son/daughter’s participation in the activities of which I have received notification. I will encourage my son/daughter to participate and co-operate with the leaders and other participants.
  • I understand that it is my responsibility to drop off and pick up my son/daughter at the designated times.  If my son/daughter is over eighteen and driving themselves, I will nominate another adult to sign them in.

Please note that checking the box and entering your name constitutes a legal agreement between St Hilary’s Anglican Church and yourself.

  • I consent to participation in the activities of which I have received notification. I will co-operate with other leaders and participants.
  • I understand that it is my responsibility to organise my own travel to and from the venue.
  • I have completed all the relevant safe ministry checks required by St Hilary's, the Anglican Diocese of Melbourne and the Department of Justice and have checked that they are currently up to date.
